Canadian researchers create list of 15 never events

A new report from Health Quality Ontario and the Canadian Patient Safety Institute details 15 preventable events that should never happen in a hospital. The report, Never Events for Hospital Care in Canada, highlights events such as surgery on the wrong body part or wrong patient or conducting the wrong procedure; wrong tissue, biological implant, or blood product given to a patient; and unintended foreign object left in a patient after a procedure.
A Never Events Action Team was formed with representatives from several groups to form a consensus on the top priorities for Canadian never events in healthcare.
