ECRI: Alarm hazards, data integrity, violence top patient safety concerns list

Alarm hazards, data integrity, and patient violence top the ECRI Institute’s list of top 10 patient safety concerns for 2015. The list is compiled by ECRI Institute Patient Safety Organization (PSO) and based on patient safety event reports, research requests, and root cause analyses submitted to the PSO.
New to the list this year are alarm hazards, mix-up of IV lines leading to misadministration of drugs and solutions, failure to properly conduct independent double checks, inadequate patient handoffs, and medication errors related to pounds and kilograms.
