Healthcare leaders discuss report on decreasing readmissions

A panel of healthcare leaders met recently to discuss a recent Robert Wood Johnson Foundation (RWJF) report citing that there has been little progress at reducing hospital readmissions for Medicare patients, and how communities and hospitals are coordinating care for patients leaving the hospital to reduce avoidable readmissions.
