As more Americans find themselves on more medications, and as these patients find themselves in the hospital for a variety of reasons, the process of medication reconciliation continues to be a problem without a perfect solution.

Baystate Health, a healthcare system based in ­Springfield, Mass., saw an opportunity to increase ­pharmacy's involvement in medication reconciliation in a single unit, and its experiment netted a significant ­success for the organization.

The organization opened up the Baystate ­Orthopedic Surgery Center (BOSC), an outpatient surgical center, in late 2011. The center approached the pharmacy about the possibility of taking over the medication reconciliation process.
