As more Americans find themselves on more medications, and as these patients find themselves in the hospital for a variety of reasons, the process of medication reconciliation continues to be a problem without a perfect solution.
Baystate Health, a healthcare system based in Springfield, Mass., saw an opportunity to increase pharmacy's involvement in medication reconciliation in a single unit, and its experiment netted a significant success for the organization.
The organization opened up the Baystate Orthopedic Surgery Center (BOSC), an outpatient surgical center, in late 2011. The center approached the pharmacy about the possibility of taking over the medication reconciliation process.
