CMS proposal would give patients a say in discharge process

To help reduce adverse events and readmissions, a rule proposed by CMS and published this week would revise the hospital discharge planning process to give patient preference greater consideration.
The rule would implement the discharge planning requirements of the Improving Medicare Post-Acute Care Transformation Act of 2014, which requires facilities to use data on both quality and resource use measures to assist patients during the discharge planning process. This includes taking patient goals and treatment preferences into account.
